Physical Therapy Evaluation — $150
Per primary condition

Physical Therapy Treatment — $100
Per visit for one primary condition

Shockwave Initial Visit — $125
One treatment area; screening and treatment if appropriate

Shockwave Follow-Up — $100
Per session for one treatment area

Lumbar Mechanical Traction — $50
Per separate visit after evaluation

Cervical Mechanical Traction — $50
Per separate visit after evaluation

 

One Primary Condition Per Visit

Related areas, such as neck and shoulder or low back and hip, may be addressed together at your therapist’s discretion within the same scheduled visit, with no additional condition fee.

Unrelated conditions, such as an ankle injury and shoulder pain, generally require separate appointments. A new unrelated condition requires its own $150 evaluation before treatment. Any approved extended appointment and fee will be discussed before scheduling.

 

Specialty Services

Shockwave is priced separately from standard PT. Any separate full PT evaluation or additional treatment area and its fee will be discussed before booking.

Traction requires a prior PT evaluation and therapist approval. Traction provided within a standard $100 self-pay PT visit is included in that visit’s price. The $50 rate applies to a separate traction visit.

 

These rates apply to our Sarasota clinic. Insurance coverage and patient responsibility are confirmed separately. Mobile PT and functional capacity evaluations have separate pricing.
